North Jax Baby Dedication Request Form

We are excited that you want to dedicate your child(ren).

Typically, baby dedications are scheduled for the first Sunday of the month
or Mother’s Day and Father’s Day.

Please submit your request at least two weeks in advance. Include a
high quality head shot photo of your child for the church screens (to myrac@njbc.org ).

Parents must attend the class before the dedication Sunday. To register for the class, please sign up here.

You will receive an email confirmation once you are scheduled.
